What should a letter to the Apple developer program look like when registering an organization?

Let me know who has come across.
I register an organization with Apple developer.
Letter Received -
"In order to proceed with registration, we require that you provide Apple with a letter on your organization's official letterhead that includes the following information:

1. State that you have legal authority to bind your organization to all legal agreements submitted on behalf of the Developer Programs Apple

2. Indicate that your LLC intends to participate in the Apple Developer Programs

3. Include your registration number 11111111.

4. Include your name, title, signature, and contact information."

Does anyone have examples of answers? It would be great.
What should be the answer to these points? Should the first paragraph contain our internal document / power of attorney, for example, the number and date of the power of attorney?
How do I indicate that our LLC organization intends to participate in the Apple Developer Programs?
