What gem would you recommend for a simple survey?

We need a gem that will display simple polls on the site like:
Do you like today's weather?
(o) yes
(o) no
(o) I don't know , I
looked at a lot of gems, but I didn't find something simple and satisfying. I don't want to reinvent the wheel.
