What are the systems for recording operational risk events?

Hello!
The task arose of creating a system for recording operational risk events and, accordingly, losses from these events.
In principle, such a system is very similar to the issue tracker system in IT. But there are a number of features:

  • Each application must have at least 2 multi-level classifiers: an event classifier and a loss classifier. For example, an event is classified as External threat (level 1) -> Illegal actions of third parties (level 2) -> Theft (level 3). And losses are classified as Material losses (level 1) -> Equipment losses (level 2). Each parent level has its own list of children.
  • The application must be processed according to a simple workflow: Receipt of the application (automatic or manual entry) -> verification and confirmation by the controller -> distribution of the application to the responsible person or group of people -> performing actions on the application and setting the mark of completion -> confirmation by the controller. It is clear that the first check can refute the application, and the last one can be returned for revision.
  • The application must contain special fields such as: estimated losses; expenses for restoration of activity; the expected period of restoration of activities; organization to which the risk is assigned; department of the organization to which the risk is assigned; date of the event, etc.
  • The application at each stage of processing should hang no more than N days. If the deadline is exceeded, the status of the application is raised, a note is made about the delay and the information is sent to the head of the department, whose employee delayed the decision.
  • Well, if the system integrates with something like AD, LDAP, Lotus, so as not to enter users and the organization structure manually

I also take risks now. Maybe the Bitrix Portal will suit you. Take the version with business processes.
With the help of business processes, you can set up a process chain, an application form, etc.
A large number of settings can be made manually. And something will have to be finished (a php programmer will be needed in this case).
