Returning to IT after a pass, what to write in a resume?

Hello, I'm a PHP developer with 4 years of experience. At the previous company, I signed an NDA, according to which I can’t even disclose the name of the company. Also, due to certain circumstances, for the last 3 years I have been working unofficially in different places (part-time jobs) not related to IT at all. During these 3 years, I was sometimes self-studying in PHP Web Development, but not regularly, rather not in order to increase the level of knowledge, but in order not to forget what I knew. Now I want to try to get back into the sphere of PHP development.
The question is how to correctly indicate the history of experience in a resume - where and how long did you work?
There was a gap of 3 years - the last specified work will be for 2016, and then it will be empty?
And also how to indicate exactly where I worked as a PHP developer before? Because I can't write the name of the company I worked for.

I don’t know, write another company in the name, and at the interview specify that you can’t give the real name. The only thing that would interest me is a long break in development.
If you worked part-time, you can specify including development. Otherwise, it will give rise to many questions.
For what reason? Are you not interested? Why did you decide to return? Long back or what? And HR'a will cling to the eye for this, I think, when reading your resume.

And what's the problem with writing the name "Secret" LLC? No one will find out whether such a firm exists or not.
In the "what you did" section, simply indicate what databases and technologies you used.
