Reliable proxy service?

Hi all!
Please advise a reliable proxy service for quiet anonymous surfing around blocking for a regular user:
1) the location of the USA;
2) without restrictions on media services (pandora.com, vimeo, youtube, etc.), on any sites, any restrictions on surfing and downloading are unacceptable.
3) monthly, quarterly, annual payment;
4) I do not require a static IP;
5) type https, not vpn - I will add foxy-proxy to the browser plugin, which manages proxies on the fly;
6) I'm not going to compromise the proxy myself: not for spam, not for fraud, not for anything criminal.
What are the prices, what suppliers?
