Are there open source form builders in php that display entities from the database (mysql). Task: to give the user the ability to add various types of fields to the entity, with the corresponding addition of the field to the database.
They are considering different options, for enlightenment in this matter, so as not to reinvent the wheel.
What would you like:
- Config field types to add;
- Validation from the same config;

Something simpler than http://wufoo.com to use for the user and with the ability to see the server side of the code.
