OpenVZ disk usage 100% what's the problem?

Good day! Very often they began to observe such a picture on the OpenVZ host node, RAM 64Gb, Swap 32Gb, SATA RAID10 + 100Gb SSD Cachecade (raid 1)
About 60 containers, mostly web servers.

LA jumps sharply and there is a failure in Cached memory. Disks start working at 100% for reading.
VSwap enabled, latest kernel, CentOS hostnode.
We have been scratching our heads for the second week, we cannot understand the source of the problem, I will be glad to help
