You can create a PDF anywhere and any way you want by outputting to a PDF printer from any editor (Inkscape, Illustrator, or even MS Word). Next, Acrobat Pro is taken, not Reader, and in it you create form fields in the right places. Templates and scripts can be attached to the form.
