I also broke my whole head why the moderation button does not appear. As a result, it turned out that the button appears only on new comments from third-party visitors. Those comments that were made before the final editing of the settings on the page and in the application settings, and moderators, facebook are ignored.

It turned out the reason was as follows: my site is multilingual, and in order for all comments to be visible in all languages ​​of the same publication, I gave a value, that is, I did not specify the publication language. The idea turned out, but over time, the moderation tool no longer worked. More precisely, the text "Tool for moderation", which was before, was not visible. It turned out that in addition to specifying the app_id, you also need to specify the og:url value and this value must match the data-url value and must be the same as in the browser line, that is, you cannot change the actual link. I set the same values ​​​​everywhere, the text immediately appeared, plus a separate comment moderation tool works.
