Java or Php library for generating Excel contract forms from a template?

There was a task to generate several forms of contracts in an Excel file according to a template. Never had to work with something like this before. The application itself is a form for processing applications, in which requests for the provision of services from service users are generated (I will attach an approximate form of the desired result below). I'm digging towards Apache POI. I would like to ask if you have had similar problems? If yes, what did they use? Is it possible in principle to generate something similar with ready-made APIs for working with Excel / Word?
