Actually the question is in the title. A software is needed to generate the documentation for the schemes.

Probably, any data modeling tool with the possibility of reverse engineering can generate documentation according to a live scheme: ERwin, ER\Studio, PowerDesigner and many others. They differ in price and features. Of the free ones, schemaspy.sourceforge.net may suit you.
