Is there an app with a timed notification that you enter data into Google Spreadsheets when answering a question?

Is there such an application? For example asks a question in a notification. Click on the notification and answer Yes, No. And the data is entered into a Google Sheets spreadsheet.

You can make a regular reminder through the google calendar app and mark it as completed or not. Collect data in a spreadsheet via IFTTT.
Or collect a survey through google forms and a reminder through google keep. But there you have to open and send the form every time - not as easy as through a notification.
You can also build a bot for a cart through an integromat that would ask such questions at a given frequency.
