Is there a ready-made tool for creating and processing web forms (like MS Infopath) but in MySQL + PHP or Python?

They set me the task - I need to make several web forms that registered users will fill out (various forms of insurance and contracts). According to their data, commissions and payments will be calculated and much more, and as a result, completed documents (contracts, policies, etc.) will be created. In general, a trifling task. You can write a form + table + field calculation + outgoing document template for each case. But firstly, there will be a lot of forms and writing everything from (form, table, logic) for each is rather dull. So I remembered the idea of ​​​​InfoPath, which did half the work itself, and paired with Access gave almost what I needed.

Well, since the decision needs to be made within the framework of LAMP, I had a doubt, is there really still no such constructor, framework, or just a set of ready-made scripts for solving a rather trivial collection of primary data? After digging around on the net, I found form designers based on the table structure. The solution is half good, but still not comprehensive.

In general, you need a constructor / plugin for CMS / CRM or something similar to create a system in which:
1. The user could register / log
in 2. Select and fill out a form, get the resulting data and documents for printing, in PDF or just HTML format.
3. An administrator/editor could quickly create new forms and customize their processing, output of results.
