Is there a HelpDesk system with a survey system out of the box?

I'm interested in a Helpdesk system (preferably a box, not a service) with a smart survey system out of the box (it can be a separate module).
Basic requirements for the survey system: A
sane form builder
Uploading reports on users, questions, time period (something like simpoll).
