Questions:
1. Should I indicate the city if there is no city in the keyword.
2. If the keyword contains the city "Rostov", can I write "in Rostov" or "Rostov-on-Don", Yandex will pick up the occurrence of the word "Rostov", if the phrase "Rostov"?

How do you feel about using templates ?
For templating, you will have to manually give all the keys the proper form, including the correct cases, numbers, prepositions, conjunctions. Well, grouping is important. However, due to "few impressions", segmentation is important anyway. Yes, and for the relevance of ads too ... or are you going only for HF-MF according to the 1-1-1 scheme?
In any case, the city can / should be indicated in the clarifications, the text of the ad, or, as they said in the second heading.
2. Direct does not take into account prepositions and cases (unless you explicitly order it to take it into account). So yes, you can safely use cases. And yes, Rostov-on-Don includes Rostov in any case, so it will be shown.
A nuance for the future: Rostov is also the plural of the word Growth. Therefore, there may, although unlikely, unpleasant surprises left requests. But I have no idea who asks Fitness Growth Trainer :)))

As for the city, what are you worried about? That ads will appear in the wrong region or that they will be less relevant?
