It means when you start the browser itself.
If for, then how?
Is it possible to launch an application from a shortcut on the desktop?

As far as I know, all applications start automatically if they are enabled (an example of this is Hangouts). About labels here .

Yes, it's possible. Use the background page - and do whatever you want there at startup.
Launching from a shortcut is also no problem. Any extension has a URL - you just need to make a shortcut to the browser, giving it this URL as a parameter.
PS It is also possible to autostart the extension when the user logs in - see background permission.
