Installing unapproved updates. How so? Why?

Good afternoon!
Help to understand the problem:
There is a network on Windows Server 2016 (only they are online), there is WSUS. Update installation policy configured:

Updates on WSUS are approved manually. But for some time now, updates began to pop up on the servers about the need to install updates.
Although they are not yet approved for installation. How so? Why? Previously, this was not the case on Windows Server 2012. As an example: KB4461600 - the update is not approved, but the server is asking for installation.
