1) Give the command you are running?
2) "ERROR: frame= 2" Error on the second frame? But what if you try to convert another file of the same format, will it work? If yes, then the problem is in the source file.
3) I could be wrong here: ffmpeg has the -v debug option to display more detailed information. Run a command with it, possibly determine the cause of the error
