I can't format the flash drive, it says the drive is write-protected. How to be?

I tried to create a bootable flash drive, Mac OS Yosemite as a hackintosh with the help of the Unibeast program, everything went fine, at the end the program gave an error and the flash drive stopped displaying. I inserted it and the USB flash drive became visible under Windows and when determining windows, it asks to immediately format the USB flash drive or otherwise it will not be possible to use it. I format with the help of Windows "Explorer" freezes completely, after 30 minutes the error "Windows cannot format this device." I tried a bunch of programs and after all, I try in the CMD diskpart utility I write clean and a message is displayed that the flash drive is write-protected. What's the matter and how to be?

Try formatting/clearing the contents NOT on Windows.
View service tools and manuals for them on flashboot.ru .
Disassemble and close contacts (which ones can be found there).
If it does not help, make a keychain (you can not put the case back, but pass the ring through a pre-drilled hole in the memory controller).
