How to use one feedback form on different pages?

There is a website with feedback forms, 2 pcs.
Plugin Contact Form 7 is used.

Task on 5 pages the form is sent with the subject of the letter: A.
On 5 other pages the form is sent with the subject of the letter: B.

How to solve such a task? Most importantly, one form is used, duplication of forms is prohibited.
I tried to write different tags from the plugin documentation in the "Subject" field, but for example _post_title does not work, because the form is in a modal window.
