How to set up Mikrotik for IP telephony if there is one Internet provider and another IP telephony?

Hello dear, dear forum users.
There is a task to set up telephony on Mikrotik. I myself have not been able to set it up yet, I have not figured it out.
There is Mikrotik equipment, the Internet provider DOM-RU enters the first port. Second LAN port. Third port for WiFi. In the fourth port, I connected the cable coming from the equipment of the provider that provides telephony, its network (10.186.19.0 - Gateway 10.186.19.1/30). And in the fifth, the PBX itself, its network (192.168.100.150/24 - gateway 192.168.100.1). Now I can ping the gateway of the provider providing telephony 10.186.19.1 from the PBX itself. After talking with the guys from the telephony provider, I was told the following, in order for everything to work, you need to achieve a ping to addresses (10.186.0.34 or 10.186.0.35), and their telephony server is 10.186.0.32/28, but it is hidden and cannot be pinged. How can I configure Mikrotik so that it goes through 10.186.19.1 and hits the 10.186.0.32 network? There is a diagram of how I see the entire data path.b80fd378460f448fb229e3ab864baa16.jpg

The problem with setting up Mikrotik is solved, all the rules are attached in the last 4 screenshots. These settings helped. Calls began to go in both directions and the voice is also heard in both directions, as well as the gap after the call is turned off ends correctly, does not hang, but is reset immediately after the call ends.
Having the opportunity, I want to thank Dmitry Shitskov, as he directly helped in solving the problem with setting up Mikrotik. Dmitry, thanks for the instructions and directions in the right directions !!! For those who are interested and maybe someone has a similar problem or even an identical PBX, follow the post, now we are trying to configure the Panasonic KX-NS500RU PBX itself. At this stage, Mikrotik was tested using MicroSIP version 3.12.3. Topic closed Automatic telephone exchange is configured. NAT was enabled in the PBX setup and the voice went to the outside world, we were heard.

Have you tried the "vlob" option? Register a route to 10.186.0.32/28 through the gateway 10.186.19.1?

/ip route add dst-address=10.186.0.32/28 gateway=10.186.19.1
