How to save additional information in Google Forms?

My web application has a link to a Google Form for a user to fill out a form. How can I save additional information about the url from which the user went to fill out the form so that the user does not see it?

In short, or embed the form in an HTML page, everything is obvious here. Or generate an image for tracking as described here , then add this image to the form.
