How to make an order form for auto transportation on Wordpress?

Hello dear friends!
Please tell me how to create a similar site kiwitaxi but on Wordpress.
Interested in the organization of the order form, with the help of which it can be done?
As you can see, the client selects point A and point B, and the appropriate carriers are selected for him.
I just can’t figure out how to do it at all, I tried it through the Custom fields, but it didn’t work out, and I also need the Receipt to be sent to the customer by mail.
