1) create a 500 by 500px document
2) create an almost black layer.
3) add Gaussian noise to it to taste (filter)
4) make an inscription and colorize the code in the desired colors
5) convert the text to curves - (this way texts are better deformed than when they are transformed with a text object, letters always jump)
6) to the selected layer with curves apply a perspective transformation by pulling the corners in the desired direction.
