How to make a web form?

There is a Wishlist to formalize the transfer of data when ordering a pass.
Now how - you need a pass - you send the data to the e-mail in the form.
In view of the fact that people are different, a problem arises - someone was written down incorrectly (for example, patronymic Mudzafirovich), something else ... But someone forgot or mixed up something in places ...
In short, we need to put things in order.
What you want:
1. That IT was not on the user's computer (but you can, of course).
2. When you click on the tab, a form the size of a phone and several fields (last name, first name, purpose of arrival, etc.) flies out.
3. When you click "send", an e-mail is essentially sent to a predefined
address
. ..all
How to solve the issue when using the site is clear to me. But how without it - I do not know.

Without a website, does it mean without a server processing it all? Can be done with arduino

Why the difficulty?
Use hosting. Only two files and the task is solved, even one can solve all this.
The form.php file, where the form is located and the form handler itself.
As a result, the form is NOT on the user's computer and the user can fill in the fields without errors in the middle name.
Access only logged in.
For the sake of one form to invent something... I don't see the point.

Yes, at least write in PowerShell , you can then compile it into an exe. And if you know how on the web it's not easier to set up a small server inside and do everything according to a familiar pattern?

Use an email client. Do you have a task to send mail?
Make a letter template, fill it out and send it to the right address.
