How to make a questionnaire in a letter?

User1 sends a letter to User2's mail, in response, the autoresponder sends him a message with fields for filling in data: full name, phone number, etc. After that, a letter with data goes to User2's mail.

  1. Is it possible to do such a filling without following an external link like Google Forms? Will Gmail or Google Chrome be banned?
  2. How much would it cost to set up such an implementation?

Within the limits of the company on corporate exchange it would be possible to make such through outlook forms. There are a lot of things that can be done in terms of paperwork.
On the random Internet, you can only write the text of the questionnaire and ask to fill it out according to some format, for example

And then catch such answers and parse on your side. But client-side response validation won't work.

It's easy to do an auto-response through sieve, but I can't imagine how to proceed. In a response letter there can be no forms, no scripts (and if there are, they will easily be cut) - there will be just text. You can ask to fill in the text fields according to some format.
And that's it. Even html is often prohibited in corporate mail.
