How to make a feedback form in an email newsletter?

Unfortunately, email forms are considered bad practice and don't work for most clients for security reasons. Moreover, it is highly likely that your letter containing the form will be perceived by the spam system as potentially malicious and will not reach the addressee at all.

You can add a form to a letter using AMP technology , although not all mailers support it (Gmail, Outlook, Mail-ru and Yahoo already definitely support it), for the rest you need to test it. AMP is very good at dealing with subscriber engagement issues. Such letters allow you to fill out a form directly from the letter, leave a comment to confirm the reservation, view your list of favorite products, for example.
