To understand English, it is best to watch various speeches on generalized topics. I would recommend TED conferences , where you can find English-language talks on any topic of interest. First you can watch with Russian subtitles, after the meaning of the words is clear - in English, then remove the subtitles for a certain amount of time (the longer you learn the language, the more minutes). So you can pump your vocabulary well, and the learning itself will be exciting.
Spoken English takes practice. No course will teach you how to speak the way you need. Look for resources where people are ready to communicate for language practice, look for conversation clubs in your city. You can also write your own mini-blog on lang-8.com where people correct your mistakes in the text (if, of course, you also correct posts in languages that you know).
