How to increase server response speed when caching NGINX with cookies?

Now the project uses the PHP-FRM7.0 + NGINX bundle.

We look through the Yandex webmaster, the server response speed is 500-600 ms. This is a lot.
After including cookies in ignore (proxy_ignore_headers Set-Cookie), we get a response an order of magnitude faster - 20-30 ms.

How to configure NGINX to not ignore cookies and increase server response speed?
