How to get this kind of blur in photoshop?

So it goes.
I tried all the blur tools in Photoshop, but could not achieve this result:

Is it even possible to get such an effect?
When I dabbled in video editors, there (like in Finul Cut Pro, or in a related one - there was a set of programs for poppy) there was a feature, you could blur (or add blur elements) in the form of hearts, stars and a similar beard.
