How to fix the layout of the form?

Good day to all.
I have a form and a map on the same page along with bootstrap-datetime-picker. And it somehow completely broke for me: https://codepen.io/slandshow/pen/EdpjaR
Can you help me fix the layout so that the form on the left fits normally in the container and does not interfere with the map (the map itself slid down)?
