How to find out which disk is not working in software RAID5 and how to replace it with Windiws 2012?

Hello. There was a problem with RAID5. Prior to this, it does not work with such raids, only with a mirror. The point is this. There is a RAID5 of 6 red WD disks of 3 TB. It hangs several bases with a total volume of 4 TB. In Disk Management, it tells me that disk 5 (the last one since numbering from 0) with exc. sign and inscription on the entire raid Failed redundancy. According to the logs, "An error was detected on the device \Device\Harddisk3\DR11 during the paging operation."
