How to display the form for adding material on MODx 1.0.12?

Good afternoon.
I got a site for revision on this CMS. She had no experience. It is required to display the form for adding Reviews. The resource itself Reviews is already there. Several have been added through the admin. The point is small, display the form for adding on the reviews page.

Docmanager is taken from here https://code.google.com/p/modxmods/source/browse/D... and lies in the created under it to the assets/libs/docmanager folder
As a result, the page crashes with the following error:

In general, the questions are:
1. Why can it crash?
2. How to find out "$doc->Set('parent',60)" and "$doc->Set('template','Article')", i.e., what to enter here?
3. What else needs to be changed in the parameters?
