How to create custom application forms in OTRS5?

There is OTRS 5 Free
Tell me how to make the first field in the application creation form select the type of application from which the rest of the fields will be loaded.
For example, I want to make the user choose:

• PC repair
  
• Moving PC
  
• Access to IT services
  

and the fields to fill depend on this.
In the demo on the OTRS website, different fields are loaded depending on the queue, this is fine for me, but I can’t find how to do it
