How to create amiro feedback module?

Good afternoon!
I want to insert a feedback module with custom fields so that the response comes to a specific email. When I click insert module, I don't find the feedback module in the list.

I also found "field sets" in the menu, but there you need to select a module, whether you can create a new one or copy the Feedback module, and modify it. The feedback module can be found in the module manager, but by the way, it is absent in the menu of fields and inserting modules.
