How to conduct a student survey?
Universities often conduct a survey of students after the session.
Usually, for each “teacher-subject” pair, it is required to evaluate parameters such as “conveys the content of the discipline well”, etc., on some scale. The list of questions is the same for all disciplines.
We tried to use Forms from Google Docs to collect responses, the result in the form of a tablet is quite satisfactory and suitable for further processing. However, each group of students has its own set of disciplines and teachers, I really don’t want to prepare a lot of slightly different forms, and if you don’t prepare and drive in a general list, then a 1st year student will be able to evaluate the subject from the 4th. In addition, there is no guarantee that the student will answer the questionnaire for all disciplines covered in the semester (I assume that he will have to run the questionnaire many times for each discipline).
In general, I want to consult which of the free or inexpensive solutions can be used to survey students. The web interface is desirable but not required. Share your experience, please :) But keep in mind that end users are not technical professionals.

In each group of students, select a volunteer who will be entrusted with the task of compiling the desired list specifically for their group.
By the way, it is not clear, this list is generally called colloquially a schedule and should exist anyway.
