2) Customize the component:
2.1) In component.php, get the select fields
2.2) In component.php, send the select field
2.3) In template.php, output the select fields
3) Customize the template
3.1) Copy mail template
3.2) Change mail template to send select value
3.3) Change component.php to send custom template
4) Place copied component component on page
