How reliable is IP filtering?

Good afternoon.
The proxy is configured with a port forwarding rule filtered by IP address.
Those. connection is possible only from 1 IP address.
Filtering is implemented using iptables.

How secure is this setup in terms of security? Is there a theoretical\practical possibility of changing the address and gaining access?

Would it be safer to create an OpenVPN tunnel, for example, and let traffic through it?

Please support the answers with a description / links to the implementation of such a substitution, if it is possible in principle, which I doubt.
