How is the IP address tracked if you use a VPN?

Let's say we connected to the VPN server. We read the news on blocked resources and disconnected.
Our ISP does not know what and where we read, but it knows when we connected to the VPN server. That is, here it is, the weak point, we connected. That is, someone asked the provider for the desired IP (VPN), and the provider says this, so he connected. And then everything is clear.
And if this VPN service (server) was used by hundreds, or maybe thousands of people, how then to search?

Request logs from the VPN provider.
But in general, there are a huge number of VPN services and a lot of people use them, including for work (to connect to the company's internal network)
. Therefore, "A person uses VPN" is not a sign of violations. Moreover, using a VPN and visiting blocked resources is not yet prohibited.

If the VPN connection is broken even for a second, then your real IP will leak, there will be a leak
