How is the document layout done?

I made a website for the customer, in addition I made him a logo. He liked the logo so much that he decided to pay extra and also asked me to make a layout of the document and folder. If everything is clear with the folder, then I have a hitch with the layout of the document.
He just needs to design the appearance of an A4 sheet, namely the header of the document: logo, phone numbers, website, etc. Then, on these sheets below, he will prescribe the terms of contracts / agreements with clients. And then I stumble: how is it done at all?

As I see it, I:
- I make it a layout in conditional pdf format. There will be only design elements, without the text of the contract
- he prints it and receives a completed sheet. Roughly speaking - like stamped paper
- then, in order to print the contract, he takes his already prepared sheet and runs it through the printer again, printing the contract itself over it.
is this how it's done?

Or should I do all the work in Word at once and give the completed docx file to him? And he will already edit the text of the agreement directly in it and immediately print it, that is, there will be no "double" printing.

Do not try to guess what the customer wants from you. If it's not clear, please clarify.
This is not an exam - both of you will be better off if you, as a performer, do what is needed right away and do not waste time on what is not needed.
Take what you wrote after the word "How I see it" and ask him.

Both methods have the right to exist.
In the case of Word, you need to take into account possible incompatibility with alternative editors (for example, LibreOffice). Therefore PDF is more reliable.
Ideally, there are 2 options: "front" in PDF, and simplified in DOCX.
In Word, I did this: I added pre-prepared pictures to the headers and footers (top and bottom), + separate header and footer settings for the first and other pages. Save better in DOTX (template).

The fact of the matter is that the client can use it in different ways. Some print blank forms in the printing house and then documents on them, others know how to work in editors (indesign, acrobat pro, etc.) and draw up documents in them, others just need a Word file with ready-made headers and footers, which is printed on an old office h / b printer.
Ask the client directly where and how he will use it, and so you will just look like a competent specialist, and not someone who will do it as convenient and let the client himself suffer "what to open it in" or "how to insert a contract there."
