A very important question for me.

I paid $25 and was told to verify my identity. I entered the name that is on the driver's license.

That is:

Driver's license:

1. Serova

2. Svetlana Vladimirovna

On the form I wrote:

Svetlana Vladimirovna (Also in Russian)

In the first line of the address I wrote:

(Name of the region) Region, city (Name of the city), street (Name of the street)

Second line I left the address blank, because I think that it is not needed.

On the form, the city wrote the name of its city

. In the region, wrote the name of the region and the index in the index. (I don't think that's the point)
