Exim4 How to configure message filtering by attachments correctly?

Filtering messages by type of files in attachments is configured. When sending from a local domain address, everything works and is filtered. When receiving mail from outside, letters with unwanted attachments reach.
How to make it so that we would receive mail from the outside, taking into account filtering, and not filter mail from the local domain at all with this acl?
