Only in very very rare cases. In other words, it's not worth it.

Blah...better take care of users with poor eyesight, there are far more of them than Explorer users.
I gave up on IE support 5 years ago.

1. The audience of your site can be VERY different. I have statistics for several projects. Somewhere support for IE6 is even relevant. ;) And somewhere - all fashionable modern for some reason.
2. Depends on the development budget, and the cost of losing a visitor to you. Again, this varies from project to project.
3. I wouldn't. In order to minimize the cost of development. For in the most general case - this allows you to save a lot.
