Can you suggest online databases?

There is a task:
To get a database of employees.
Ideally something similar to google forms. So that there is a simple input form, the ability to build all sorts of graphs, the ability to work together.
Options that came to my mind:
1. Those same google forms;
2. google fusion tables. As I understand it, this is something similar to what I need, but data entry is like in spreadsheet editors? It's also embarrassing that most of the examples come in conjunction with maps. fusion tables is somehow sharpened for maps?;
3. Have your own small server with debian and apache. It was set for moodle. Accordingly, there is mysql. Maybe there is some kind of web application that serves such purposes? Input form, mysql database, beautiful graphs, tables, reports...
I apologize for the chaotic question, I just have not been able to formulate it for myself for a week now. I hope the idea is clear.

Nothing is clear)) How many employees, what information about them is in the database, should they all be included in it or only the boss, whether an interface is needed to display information from the database for "non-programmers", etc.

More than a thousand employees. The information is varied. A lot of information. Each employee must have a record in the database. You definitely need an interface not only for output to non-programmers, but also for input by non-programmers. :)
