Can you share a currency transaction statement with Upwork?

When transferring funds from Upwork to an IP account, you must go through currency control and fill out a certificate of currency transactions.
VTB example: www.vtb24.ru/company/currency-operations/Documents...
Does anyone have a completed certificate for this Upwork site?
Display of works, amounts, taking into account exchange commissions, etc.

Connect your client bank. In a client bank, after selecting an account, most likely, you will have to fill in only 3 fields:
Invoice date (or work acceptance certificate), Invoice number (through a slash contract number), and Currency transaction code. Code 20200 is suitable for dress code (a non-resident pays for services performed by a resident after they are completed). For the future, code 20100 (services, advance payment) may come in handy.
If there is no contract number, then you can only specify the date, for example, BN / 20.20.2016
Well, it's best to pay the bank a processing fee once. In normal banks, these are tenths of a percent of the transaction amount. You will be given a copy of the certificate, and next time you can fill it out simply according to the model.
