We need a program to record data on ultrasound of patients.
Patient list - view and add ultrasound data for a patient (one to many).
Ultrasound data - date and text.
Patient - last name, first name, patronymic, year of birth. Patient search.
Maybe there are some ready-made programs for creating forms, working with a database, or writing it yourself? As a sqllite db?
I looked from the ready-made mydataexpress, in principle it creates a form, only the search is not very convenient.

There is a DICOM standard. Theoretically, the software in the device should allow all this to be filled in and uploaded to the DICOM server.
