A program for compiling arbitrary forms and recording data?

I need to create a form with a certain set of fields and periodically fill it out by adding new records to the table (I don't care what format it will be in - the main thing is that the program displays it). Does such software exist? At least under Windows. Well, or at worst, some kind of Internet service? It’s just that only I will fill out this form and the data is a little personal. Purely so as not to forget anything ... You can, of course, create a spreadsheet in Excel. I've already tried that - it didn't last long. Too many hands to score. And I want to poke the mouse and choose from comboboxes ...

In Google Sheets (and in Excel itself, probably), you can create comboboxes with a choice of several options. And there are formulas, and graphs. You just need a little creativity, and there is a lot of functionality there. :)
