how to fix the contact form and leave only the name and text of the error (well, the submit button).
Ah and soap likely should in the same form right. there done through the model. so I don’t have such a model in the database

This model is not bound to a database table. Look, it extends Model, not ActiveRecord.
Edit the model and edit the view and that's it.
