Why is Nominatim not working?

How to properly initialize a database with Nominatim?
It seems to have launched setup.php. It created the tables, then imposm loaded the data dump, but then Nominatim can't query the database: ""relation "placex" does not exist""

setup must be run with the --all key and wait a week.
