Why doesn't hook_form_alter drupal 7 work (website goes down)?

When using myTheme_form_alter, the front-end of the site goes out, i.e. does not show up at all. Moreover, the problem occurs even if there is no code inside the hook. What could be the problem? How to solve it?

Look at the Apache error.log, it should say what causes the error and causes the script to stop.
